Medical/Dental Insurance

Our practice is considered Fee for Service which means we do not participate with any Medical or Dental Insurance Companies. The patient is personally responsible for any and all charge incurred.

Any patient visiting our office for TMJ or Sleep Apnea related appointment should bring their Medical Insurance card with them. Our Medical biller will submit the consultation and appliance visit for the patient's reimbursement.

Any patient visiting our office for a Dental related appointment should bring their Dental Insurance card for the Dental Biller to submit for any major services the patient might incur.

All patients should be aware of they do not have out-of-network benefits there will be no payment available for any service done in our office.

In House Labs

The Gelb Center has a Dental Lab on the premises. Our lab is equipped to fabricate all TMJ appliances, Most Sleep appliances, Whitening trays, all phases of Denture fabrication and repairs and Temporary crown and bridge work.

The Lab hours are Monday - Thursday: 8 am - 5 pm & Friday: 8 am - 4 pm.
